When a drug developer trades below the cash on its balance sheet, the market is saying it expects much of that cash to be spent without a return. Sometimes that view is right. The Desk screened liquid US listed healthcare names between $50 million and $500 million for net cash above market value, then read each company's latest filings for cash, liabilities and how fast the money is going out.

The four below are ranked by cash as a multiple of market value. Cash is a starting point, not a floor: none of these companies is planning to return it to shareholders.

Screen: stockanalysis.com market data, pulled October 20, 2026. Balance sheets: each company's Form 10-Q for the quarter ended June 30, 2026, plus later 8-K filings named below.

The Desk's Read: Neutral

Trading below cash is a measure of doubt, not a margin of safety. For each name the useful question is how many quarters of spending the cash buys and what has to happen before it runs out: pack growth at LENZ, TONMYA revenue at Tonix, a Phase 3 start at Upstream, and progress on ATR 1072 at Atrium.
